Carolina Medical Homes is a primary care case management program that is contracted with the South Carolina Department of Health and Human Services. CMH is a medicaid program that pairs patients with a primary care physician and helps them develop a one-on-one relationship. CMH provides a nurse hot line 24 hours a day and on going care for those affected with diseases such as asthma and diabetes.
